Louisiana Rural Health Transformation Program

Executive Order JML 26-032 establishes the Louisiana Rural Health Transformation Program and creates the Office of Rural Health Transformation and Sustainability within the Louisiana Department of Health. The program aims to address Louisiana's ranking as the 50th least healthy state by pursuing three strategic priorities: improving population health outcomes, expanding access to care, and strengthening system capacity.

Affected parties include rural healthcare facilities, healthcare workforce training programs, and approximately 1.1 million rural residents across Louisiana's 37 rural parishes. The program targets elevated chronic disease rates (hypertension at 44%, obesity at 43%, diabetes at 17%) and maternal mortality 14 percentage points above the national average. Implementation will involve partnerships with educational institutions for workforce development, creation of unified health technology ecosystems, and coordination with CMS for the federal Rural Health Transformation Program grant.
