Summary

The Wisconsin Office of the Commissioner of Insurance (OCI) published its March 2026 administrative actions list detailing enforcement measures taken against insurance agents and license applicants. Actions include license revocations, application denials, and forfeiture orders totaling at least $1,500. Violations cited include tax delinquency, unreported administrative actions from other states, criminal convictions, and failure to disclose pending charges.

What changed

OCI published its monthly administrative actions list for March 2026, documenting seven enforcement actions against insurance agents and license applicants. Actions included: license revocation for Theresa A. Buehring (tax delinquency); denial of applications for Murat Dorkan (unfair practices/fraud) and Jordan Ingalls (criminal convictions); and forfeiture orders for Charles Elliott, Frank Guzman, and Jontai Holmes ($500 each, for failing to disclose out-of-state administrative actions and violating OCI warning letters). Jesus Gutierrez received a one-year probationary license.

Insurance agents and applicants in Wisconsin must ensure they timely disclose all administrative actions taken by other states and comply with any OCI warning letters. Forfeitures are deposited in Wisconsin's Common School Fund. Copies of full administrative orders are available at ociaccess.oci.wi.gov/OrderInfo/OrdInfo.oci. Entities with questions about OCI enforcement actions can contact Lauren Van Buren at (608) 261-8283.

What to do next

  1. Review all administrative actions from other states and ensure timely disclosure to OCI
  2. Comply with any warning letters issued by OCI before engaging in further licensed activities
  3. Ensure all Wisconsin state tax obligations are current to avoid license revocation

Penalties

Forfeiture penalties of $500.00 per violation; license revocation for tax delinquency; license application denials for fraud/unfair practice findings and criminal convictions

Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Copies of the administrative orders may be viewed online at ociaccess.oci.wi.gov/OrderInfo/OrdInfo.oci.

Any forfeitures paid are deposited in Wisconsin's Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

If you have a question or complaint regarding an insurance company or agent you can use the online complaint form on our website, oci.wi.gov, or contact OCI at 1-800-236-8517.
