This report summarizes new, interim, and final actions taken by the Oregon Medical Board during the reporting period. Notable entries include a non-disciplinary Consent Agreement for Re-Entry to Practice for Brigitte Diane Buehler, PA requiring 2,000 hours of supervised practice with a pre-approved physician mentor, and an Order Terminating Consent Agreement for Barbara Ayres Ball Trask, PA. A Notice of Proposed Disciplinary Action was issued to Sajad, MD, though this represents a proposed action and not a final Board determination.
