DS035064, of Drums, Luzerne County was reprimanded and indefinitely suspended and she shall pay a civil penalty of $5,000.00, costs of investigation of $8,280.11, and complete 8 hours of remedial continuing education because she committed unprofessional conduct in that she failed to provide records to previous patients. (07/25/25) Tioga County Stephen Joseph Ollock, license No. DS038254, of Mansfield, Tioga County, was indefinitely suspended, with said suspension immediately stayed in favor of no less than three years of probation, based on his inability to practice dentistry with reasonable skill and safety to patients.

(07/24/25) PA Board of Professional Engineers, Land Surveyors and Geologists PA Board of Funeral Directors Raphael M. Hunt-Irving, F.D., license number FD014558L, of Chester City, Delaware County was actively suspended for being found guilty of four felonies and crimes of moral turpitude. (7/10/25) PA Board of Landscape Architects PA Board of Massage Therapy Rick A. Long, Jr., license no. MSG012433, of Coraopolis, Allegheny County, had his license indefinitely suspended retroactive to April 7, 2025, because he is unable to practice massage therapy with reasonable skill and safety to patients and for violating a Board order. MSG009683, of Duncansville, Blair County, was levied a civil penalty of $1,000, shall pay the costs of investigation of $551.16, shall complete 6 hours of continuing education on the topic of recordkeeping, and shall have a public reprimand placed on her permanet record with the Board. Pecora, notary commission no. 1338567, of Nesquehoning, Carbon County, voluntarily surrendered her notary commission because she notarized a document without requiring personal appearance and failed to record in her journal all required information for all notarial acts she performed. (8/11/25) Lehigh County Lamia Afif, notary commission no. 1300546, of Catasauqua, Lehigh County, was suspended for 12 months with the entire period being stayed on probation; ordered to pay a probation assessment of $600; and an administrative penalty of $1,800 because she charged fees not consistent with the fees permitted by the Department; failed to provide an itemized receipt identifying the fee charged; failed to record any notarial acts she performed in a notarial journal; and accepted Accelerated Rehabilitative Disposition (“ARD”) for a second degree misdemeanor. RS378896, of Drums, Luzerne County, had his application for a real estate salesperson license granted and placed on probation for a period of time to run concurrent with applicant’s criminal probation in Luzerne County that is scheduled to end on April 2, 2029. (08/05/25) Renee Friedel, notary commission no. 1265108, of Delta, York County, was ordered to pay an administrative penalty of $500 and complete six hours of remedial education, because Friedel failed to record all required information in her journal of notarial acts. (08/27/25) Elk County Taylor, Leeann Thiers, license No. PN319615, of St. Marys, Elk County, had her license suspended for no less than three years, retroactive to May 3, 2024, for violating a previous board order. (8/26/25) PA Board of Physical Therapy PA Board of Podiatry PA Board of Psychology Jeffery Leoni, license nos.: PS016429 of Sinking Spring, Berks County, agreed to the indefinite suspension of his psychologist license, because the Commonwealth alleges that by virtue of pending criminal charges, Leoni is an immediate and clear danger to the public health and safety. RB068888, of Wexford, Allegheny County, shall jointly and severally pay a civil penalty of $5,000; costs of investigation of $190.05; and Bickel Tauber shall complete a three-hour remedial education course, because they engaged in or conducted the business of a broker without being licensed; and paid or accepted an undisclosed commission or profit.
