Centre County U.S. Accounting and Tax Services, LLC & Leonard E. Stefanko, license no. CA007160L, of State College, Centre County shall jointly and severally pay a civil penalty of $5,000; and U.S.

Accounting shall immediately cease and desist from engaging in activities requiring an accounting firm license. (11/19/25)

Kevin Chou, license no. CA060523, of Philadelphia, Philadelphia County, was ordered to pay a civil penalty of $2000; costs of investigation of $137.67; and complete an eight hour remedial education course, because Chou failed to exercise due professional care in the performance of professional services. of $500 and shall cease and desist from operating a shop without supervision of designated manager barber or other designated licensee, because Quality Cutz operated a shop without supervision of a designated manager barber or other designated licensee. (11/24/25) Blair County

Eugene I. Ehredt, license no. BL055406 AND BM094462, of Altoona, Blair County, was suspended for no less than two years, stayed in favor of probation, and shall have a public reprimand placed on their permanent record with the Board, because Ehredt was convicted of four felonies and further licensure would pose a substantial risk to the health and safety of the public. BL053455, of Mechanicsburg, Cumberland County, was levied a civil penalty of $1,500 and shall have a public reprimand placed on his permanent record with the Board, because Taylor practiced as a barber on a lapsed or expired license and lied on his reactivation application about practicing on a lapsed or expired license. BL051769L, of Harrisburg, Dauphin County, was suspended for six months stayed in favor of probation and shall have a public reprimand placed on their permanent record with the Board, because Williams was convicted of a misdemeanor related to barbering (11/24/25) Lehigh County

Major League's, license no. BO433352, of Bethlehem, Lehigh County, was levied a civil penatly

of $6,650 and shall cease and desist from employing unlicensed persons, because Major

League's employed three unlicensed persons to perform barbering services, operated without supervision of a designated manger barber or designated licensee, and failed to meet minimum maitenance and sanitation requirements. (9/22/25) Luzerne County

Scarlet Borbon d/b/a Good Faith Barbershop, of Nanticoke, Luzerne County, was levied a civil penalty of $500.00, as well as the cost of investiagtion of $126.54, for operating an unlicensed barbershop. Scarlet Borbon d/b/a Good Faith Barbershop has also agreed to cease and desist the operation of the unlicensed barbershop. (11/24/25) Montgomery County

D-Nices Barbershop, license no. BO434248, of Willow Grove, Montgomery County, was levied a civil penalty of $2,500, and shall have a public reprimand placed on their permanent record with the Board, because D-Nices Barbershop failed to employ licensed persons and failed to be at all times under the immediate supervision of a manager-barber or a licensee designated in charge of the shop. BO436286, of Norristown, Montgomery County, was levied a civil penalty of $2,500, shall cease and desist from employing unlicensed persons, and shall have a public reprimand placed on their permanent disciplinary record with the Board, because Chino Barbershop 2 operated without supervision of a designated manager barber or other designated licensee and employed four unlicensed persons to perform barbering services (11/24/25)

Navigation Commission for the Delaware River and its Navigable Tributaries Secretary of the Commonwealth - Notaries Public PA Board of Nursing Bethany Anne Hamilton, RN, license no. RN715951, of Pittsburgh, Allegheny County, had her professional nursing license automatically suspended, retroactive to July 19, 2024, based on

Courtney Nicole Mishoe, LPN, license No. PN285598, of Annville, Lebanon County, had her license suspended for no less than five years, retroactive to April 16, 2025, because she violated a previous board order. (11/24/25) Demetrius Donell Johnson, LPN, license no. PN315447, of Philadelphia, Philadelphia County, was suspended pursuant to the Order of the Court of Common Pleas of Philadelphia County Page 6 of 11

dated October 15, 2025, which the court issued under section 4355 of the Domestic Relations Code. The suspension is effective immediately. (11/18/25) Lauren Kusy, license no. RN580098, of Philadelphia, Philadelphia County, had her registered nursing license suspended for no less than three years, retroactive to October 30, 2024, because she violated a previous Board Order. Melissa Kay Nelson, LPN, license no. PN306787, of West Newton, West Moreland County, had her license reinstated to probationary status after complying with a previous board order. (11/26/25) York County

Obiageri Chukwu, license no. PN269535 of York, York County, had her license to practice as a practical nurse temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth presents a clear and immediate danger to the public health and safety. Vijay Peethambaram, real estate salesperson license number RS338509, of Phoenixville, Chester County, was suspended for no less than three years, retroactive to August 13, 2025, followed by two years of probation subject to terms and conditions, and was assessed a $6,000 civil penalty and the cost of investigation in the amount $1,152.40, for making a substantial misrepresentation; accepting a commission or any valuable consideration as a salesperson for the performance of any acts specified in the act, from any person, except the licensed real estate broker with whom he is affiliated; and performing any act for which an appropriate real estate license is required and is not currently in effect.; (11/4/25) Brett Grossman, of Phoenixville, Chester County, was assessed a $10,000 civil penalty and costs of investigation in the amount of $225.38 and was also issued a cease-and-desist orders for engaging in the unlicensed practice of the real estate profession. RS339927, of West Chester, Chester County, was ordered to pay a civil penalty of $2,800 and complete 14 hours of remedial education, because Whitnah renewed his license without completing the required continuing education for the preceding license period. Tyquann Gant, License No. RS359606, of Mountville, Lancaster County, was suspended pursuant to the Order of the Court of Common Pleas of Lancaster County dated October 29, 2025, which the court issued under section 4355 of the Domestic Relations Code.
