CY107156, of Waynesboro, Franklin County, was ordered to pay a civil penalty of $3,250 and its license was publicly reprimanded because it failed to properly sanitize the salon and used MMA and metal rasp tools in the salon. CY197446, of Mifflintown, Juniata County, was asessed a civl penalty of $500, have a public reprimand placed on their permanent Board record, and shall cease and desist from possessing metal rasp tools, because Royal Nails operated as a nail technology in a grossly incompetent, unethical, and dishonest manner through the possession of a metal rasp tool. CL1826506, of Huntingdon Valley, Montgomery County, was levied a civil penalty of $500 and shall have a public reprimand placed on their permanent Board record, because Truong practiced as a nail technician without an active license from approximately February 1, 2024 through June 26, 2024. Respondent is subject to disciplinary action under the Revised Uniform Law on Notarial Acts, 57 Pa.C.S. § 323, in that Respondent lacks the honesty, integrity, competence, or reliability to hold the office of notary public. Lackawanna County Thomas J. Miller, notary ID number 1248402, of Dunmore, Lackawanna County, had his commission revoked and is prohibited from reapplying for appointment and commission as a notary public for a period of four years, based on his failure to require personal appearance of an individual executing their signature on a record, and to maintain a journal of notarial acts. Respondent is subject to disciplinary action under the Revised Uniform Law on Notarial Acts, 57 Pa.C.S. § 323, in that Respondent lacks the honesty, integrity, competence, or reliability to hold the office of notary public. Luzerne County Franklyn K. Nunez, notary ID number 1228548, of Hazle Township, has had his commission revoked based on his admission of guilt to a felony conviction and his failure to disclose such criminal history on his application for reappointment as a notary public in 2018 and 2022, as well as his failure to maintain accurate journal entries. Respondent is subject to disciplinary action under the Revised Uniform Law on Notarial Acts, 57 Pa.C.S. § 323, in that Respondent lacks the honesty, integrity, competence, or reliability to hold the office of notary public. Darlene Mccullough, notary commission no. 1200320, of Philadelphia, Philadelphia County, was placed on probation for six months; ordered to pay an administrative penalty of $250.00; and a probation assessment of $300.00, because McCullough performed a remote online notarization prior to receiving approval by the Department as an eNotary.
