AF000299L, of Pittsburgh, Allegheny County, was ordered to pay a civil penalty of $3000 and costs of investigation of $761.12 because they engaged in the practice of public accounting in the Commonwealth without holding a current license. BL063493, of Pittsburgh, Allegheny County, had his application to sit for the barber licensure examination granted and, upon successful completion of the examination, his barber license shall be granted and then immediately placed on probation for a period of twelve months based on a criminal conviction. (11/17/25) Codell M. Daniels, license number BL054826, Pittsburgh, Allegheny County, had his barber license revoked because of his criminal convictions and failure to report those convictions. (1/6/26) Terry Williams, Jr., License No. BL058050, of Harrisburg, Dauphin County, was suspended pursuant to the Order of the Court of Common Pleas of Dauphin County dated January 12, 2026, which the court issued under section 4355 of the Domestic Relations Code. BO432568, of Philadelphia, Philadelphia County, was assessed a civil penalty of $2,500 for operating a barbershop with a lapsed and suspended license, employing an unlicensed individual, and violating a lawful disciplinary order of the Board. (12/9/25) Elk County Daniel Joseph Carr, last known of Saint Marys, Elk County, was ordered to cease and desist from engaging in the practice of chiropractic and ordered to pay a civil penalty of $10,000.00 and investigatory costs in the amount of $4,735.20, for engaging in the practice of chiropractic without a valid license and for continuing to practice chiropractic after the Board ordered the revocation of his license. CO321545, of La Belle, Fayette County, had his application to sit for the cosmetology licensure examination granted and, upon successful completion of the examination, he shall be granted a restricted license for a period of twelve months based on his criminal convictions. (01/06/26) Montgomery County Margaret Ann Marks, license no. CO154312R, of Elkins Park, Montgomery County, was suspended for failing to pay a previously imposed civil penalty. (12/04/25) Mckinley Mills, Jr., License No. CO295248, of Philadelphia, Philadelphia County, was suspended pursuant to the Order of the Court of Common Pleas of Philadelphia County dated December 18, 2025, which the court issued under section 4355 of the Domestic Relations Code. MSG011482, of Stroudsburg, Monroe County, had his license revoked, was ordered to pay a civil penalty of $5,000.00, and costs of investigation of $177.56 because Respondent engaged in sexual harassment, sexual impropriety, sexual violation, or sexual abuse; failed to use safe and functional coverage/draping during the practice of massage therapy; and intentionally exposed the client's genitals. Feeney license no. RN240870L, of Harrisburg, Dauphin County, shall voluntarily surrender her license because, Feeney was convicted of one count of Theft By Fail To Make Req Disp Funds (F-3). (1/13/26) Erie County Angela Sue Crosby, LPN, license no. PN278059, of Corry, Erie County, suspended for two years, immediately stayed in favor of probation; and must complete five hours of continuing education on standard of care and ten hours of continuing education on nursing ethics and/or professional accountability, because Crosby engaged in unprofessional conduct in the practice of nursing. CW016907 of Butler, Butler County, Pennsylvania agreed to the voluntary surrender of his license to practice as a clinical social worker because he entered a dual relationship with his clients, engaged in unprofessional conduct, and practiced below the standard of care. Susquehanna County Linda Susan Cronin, license no. CW012965, of Montrose, Susquehanna County, was placed on, suspension for a period of 3 months to be stayed in favor of probation, received a public reprimand, was required to pay a civil penalty of $1,500, and was ordered complete 15 credit hours of continuing education because Cronin failed to conform to the standards of acceptable and prevailing practice. (12/17/25) York County Antonio Luis Cedeno Madera, vehicle salesperson license number MV239036, of Red Lion, York County had his vehicle salesperson license revoked and to cease and desist due to his felony criminal conviction. (12/17/25) PA Board of Veterinary Medicine Carlton Benner Seybolt, VMD, license no. BV008923L, of Riegelsville, Bucks County, received a public reprimand, incurred the costs of investigation of $1,366.63, and was ordered to complete 10 hours of remedial education on proper surgical procedures by 05/30/26, because Seybolt failed to conform to the standards of acceptable and prevailing medical practice. (01/30/26) Cumberland County Grayson Ralph Bowers Jr, DVM, license no. BV004665L, of Carlisle, Cumberland County, was suspended for two years, with 6 months served as an active suspension and the remaining suspension to be stayed in favor of probation, and levied the costs of investigation of $4,807.97, because Bowers Jr. failed to conform to the standards of acceptable and prevailing veterinary medical practice. BV012942, of Blairstown, New Jersey, was levied a civil penalty of $7,500; costs of investigation of $1,555; and must cease and desist the practice of veterinary medicine in the Commonwealth of Pennsylvania unless and until his license is reactivated/reinstated in the Commonwealth of Pennsylvania, because he practiced veterinary medicine in the Commonwealth of Pennsylvania on an expired license.
