Florida Elevator Safety Forms and Applications
FL DBPR's Division of Hotels and Restaurants published a comprehensive forms portal for elevator safety regulation in Florida. The page consolidates forms for accident reporting (DBPR HR-7016), consumer complaints (DBPR HR-7003), license and permit applications, continuing education provider registration, variance petitions, and inspection-related documents. The division now offers online accident reporting and complaint filing through DBPR Online Services in addition to paper PDF forms. Permit holders must notify the division at least five days before initial acceptance inspections using the web form provided.
