Pursuant to Section 14 of the Act (820 ILCS 305/14), the Chairman evaluates Arbitrators seeking reappointment and may recommend them to the Governor. To assist in this process, please complete a brief survey for any Arbitrator with whom you have had direct interaction. The survey is easy to use and includes space for comments. Only Arbitrators seeking reappointment are included. Please do not submit evaluations for individuals you have not worked with.
