The service will deliver a community-based palliative and end of life care services for individuals aged 18 and over requiring domiciliary end of life care, complex symptom management and assessment of their changing condition and/or disease progression.

Palliative Care Personal Carer Support Service

The service will support the needs of family and carers of those who require palliative and end of life care by enabling them to have regular breaks and respite from the caring role at times to suit them.
