Who can use this form
You can use the online form if you’re appointed as:
- a trustee in bankruptcy or sequestration
- a liquidator
- an administrator

To use this form you’ll need the:
- name of the organisation or individual
- business address
- Economic Crime Levy registration number
- type of insolvency
- date when the business became subject to the insolvency procedure
deregistration date
