What changed

The Washington State Department of Health has taken disciplinary action against two health care providers in March 2026. Reshawn Bernard Hayes, a respiratory care practitioner, had his credential indefinitely suspended following arrests for domestic violence offenses and admitting to intoxicant consumption before an investigatory meeting. Hinda Ahmed Mohamed, a certified nursing assistant, also had her credential indefinitely suspended for allegedly borrowing a significant sum of money from a client and failing to respond to the department's inquiries.

These actions represent final disciplinary measures by the state. Healthcare providers facing similar allegations or who fail to cooperate with departmental investigations should anticipate severe consequences, including license suspension or revocation. The department encourages consumers to report unprofessional conduct and utilize the provider lookup tool on their website for credential status and disciplinary history.

What to do next

  1. Review disciplinary actions for any providers with similar names or circumstances.
  2. Ensure all staff are aware of the importance of cooperating with DOH investigations.
  3. Verify credential status for all healthcare providers employed by the organization.

Penalties

Indefinite suspension of health care provider credentials.
