Welcome to our first Wisconsin Pharmacy Examining Board Chair’s Corner.

Let me introduce myself. I am John Weitekamp, current Chair of the Wisconsin Pharmacy Examining Board. I have been a practicing pharmacist for 41 years in the retail setting. I have served on the Wisconsin Pharmacy Examining Board since 2018, serving as Chair since 2021. During my tenure as Chair, we have been able to make many updates to pharmacy rules as they affect pharmacy practice.  The latest update we have adopted was a repeal-and-recreate of Wis. Admin. Code ch. Phar 15 Pharmaceutical Compounding, Safe Handling of Hazardous Drugs and Radiopharmaceuticals which was effective as of October 1, 2025. I wanted to address some of the changes made and how they may affect pharmacy practice.

The key highlight I wanted to concentrate on was compounded GLP-1 medications (for example semaglutide or tirzepatide). Sterile compounded preparations inherently carry higher risk to patients (microbial contamination, particulates, sterility failure) and are subject to heightened regulatory scrutiny. The FDA recently issued guidance advising that compounders may not compound drugs that are essentially copies of FDA-approved drugs, unless certain conditions such as a documented shortage apply. In the GLP-1 space, the FDA has issued warnings about compounded GLP-1 drugs and made it clear that compounded products cannot simply replicate FDA-approved GLP-1 drugs when those approved are no longer considered in shortage.

If you dispense or oversee the dispensing of compounded GLP-1 preparations in Wisconsin, you must ensure that any “compounded GLP-1 product” meets all applicable compounding requirements (facility, process, documentation, verification) under Wis. Admin. Code ch. Phar 15. If a compounded GLP-1 product essentially duplicates an FDA-approved GLP-1 drug and that FDA-approved drug is not currently designated in shortage, federal law may view the compounded version as “essentially a copy” and subject to enforcement. Therefore, dispensing a compounded GLP-1 drug may carry risk of non-compliance under both state (Wis. Admin. Code ch. Phar 15) and federal law.

Phar 15 and applicable federal regulations, you help safeguard patient safety, reduce regulatory risk, and maintain the integrity of your compounding and dispensing services. If your pharmacy has switched to paperless patient education information, here are some best practice guidelines:

  • Always inform upon dispensing: With each medication dispensed, verbally inform the patient that printed medication information is no longer automatically provided.
  • Point out QR code: Direct the patient to the QR code on the label, receipt, or handout, which links to the digital patient information.
  • Honor print requests promptly: If a patient prefers printed information, provide it upon request without hesitation or judgement.
  • Be consistent and courteous: Use consistent language and a friendly tone to ensure patients feel supported and informed.
    Sample scripting:

  • General Script: "Just a quick note-our pharmacy now provides medication information digitally. You can scan the QR code on your label or receipt with your phone's camera to read about your medication. If you'd prefer a printed copy, we're happy to provide one-just let us know!"

  • For returning patients: "As a reminder, we've gone paperless with medication info. You can scan the QR code to view it on your phone. Let me know if you'd like a printed version today."

  • For elderly or less tech-savvy patients: "We now offer medication information digitally through a QR code. Just point your phone's camera at it. But if you'd rather have a printed copy, I can get that for you right away."
    Sample signage suggestion:

"We've gone paperless! Medication information is now available digitally. Scan the QR code on your label or receipt with your phone's camera to learn more about your medication. Prefer a printed copy? Just ask, we're happy to provide one!"

Please note: Sample scripting and signage are NOT required by the Pharmacy Examining Board. They're provided here as helpful suggestions to explain the transition to electronic information to patients. Each pharmacy must decide how they want to communicate this transition.

Board Member Spotlight: Tiffany O'Hagan, PharmD, MBA

Tiffany O’Hagan has served on the Pharmacy Examining Board since July 2020, bringing nearly 25 years of diverse experience in pharmacy practice. Over the course of her career, she has held roles ranging from pharmacy technician and intern to pharmacy manager and clinical account executive, giving her a comprehensive perspective on the profession.

A proud graduate of the University of Minnesota, Tiffany earned both her Doctor of Pharmacy (PharmD) and Master of Business Administration (MBA) before returning to her hometown of Oshkosh, Wisconsin to continue her practice. Her commitment to advancing pharmacy extends beyond her professional roles—she founded the Fox Valley Pharmacy Association, a local group designed to connect healthcare professionals and foster collaboration through education on emerging therapies.

Tiffany’s leadership also reaches the national stage. She has been an active participant in the National Association of Boards of Pharmacy (NABP) and currently serves as the Executive Chair of District IV, where she works to strengthen partnerships and promote innovation across the profession.

Her dedication to pharmacy practice, education, and collaboration reflects her passion for ensuring that pharmacists remain integral to patient care and the broader healthcare system. |

Newsletter Liaison Spotlight: Christa Wilson, PharmD

I have served on the Pharmacy Examining Board since 2021. I graduated from Creighton University in Omaha, Nebraska. I currently work at Lakeshore Community Health Care as the Director of Pharmacy, which serves as both a community and clinical pharmacy in northeast Wisconsin.

I serve on the PEB because I am a rule-follower and having a front-row seat to the rules of pharmacy practice in Wisconsin is a great way for me to adhere to the rules and communicate them out to my organization and colleagues in the state.  I also enjoy bringing my perspective as a pharmacist in a community health center because of the unique challenges and considerations in this practice setting.

In my free time, I enjoy watching my three children in all their sports and activities (especially the INDOOR ones!) I also love running, and my two very active and social Siberian Huskies who make great running partners when they aren’t playing with their 10 other Siberian Husky best friends.

Change in Managing Pharmacist

DSPS offers Downloadable Digital License

The Wisconsin Department of Safety and Professional Services is the first multi-occupation licensing agency in the country to offer a free downloadable digital license for its credential holders. Now, Wisconsin occupational license holders can carry their credentials wherever they go.

DSPS has posted on its website a simple guide for downloading a Digital Wallet Card.

Duty to Report Convictions

Pharmacists and Pharmacy Technicians are required to provide certified copies of the criminal complaint and the judgement of conviction within 30 days of any conviction (felony or misdemeanor). Failure to report a conviction within 48 hours is unprofessional conduct under Wis. Stat. § 450.10. Board action, including discipline, may result for any conviction that is substantially related to the practice of pharmacy. Board action may also result from a failure to timely report any convictions as required.

The Pharmacy Examining Board, in collaboration with the staff at the Department of Safety and Professional Services, can take action against its licensees to help protect the profession and the citizens of Wisconsin. Please note that confirmation of changes is not automatically provided. Legal notices will be sent to a licensee’s address of record with the Department.

Under s. 440.01(1), Wis. Stats., an applicant or credential holder has 30 days to provide notification of a change in name or address. Similarly, under s. 440.11(1m)(a), Wis. Stats. an applicant for or recipient of a credential who changes his or her electronic mail address or whose current electronic mail address becomes inactive shall notify the department of such a change within 30 days of the change.
