The Alabama Department of Public Health would like to hear from state residents about health issues they believe are the most important in their communities. All Alabamians ages 18 and older are encouraged to express their concerns and opinions in a confidential online survey.

The survey should take less than 10 minutes to complete. To access the online survey, please visit the following link.

  • Help Us Make Alabama Healthier! The 2026 Community Health Opinion Survey is a component of the State Health Assessment that identifies health issues and challenges at the community level. Results will be used to guide future public health strategies in areas where improvements are most needed. Based on this information, the department can better serve Alabama residents by addressing their highest-priority needs to improve health outcomes.
