Medicar e Guide Disclaimer: This guide covers the basics – and just the basics – about applying for Medicare. I t does not include every det ail of t his complicated subject. You can contact SHI NE (Serving Healt h Insurance Needs of Everyone) for more information. SHINE is an educational resource t hat is designed to inf orm you about the complexities of Medicare. Information was obtained via the Social Security, Medicare, and Division of Insurance websites, IRS, as well as the Medicare & You Handbook. Are you re ady to enroll in Medicare? If so, you r fi rst step sh ould be to contact t he So cial Security Admi nistration! If you’re turning 65 in t he next 3 months and not ye t receiving benefits from the Social Sec urity Administration, you will n ot get Medicare automatically! It is your responsibility to con tact Social Security if you wish to enroll in Medicare. You wi ll not receive any reminders or notifications from either Social Security or Medicare. Socia l Security Disab ilit y I nsurance (SSDI) and Medicare - If you’re under 65 and have a disabil ity, you’ll automatically get Part A and Part B once you have received 24 months of SSDI payment s. Social Security manages enrollment in Parts A an d B, and will review your records to see if you qualify for Medicare. They will determ ine if you qualify f or premium - free Part A, and w hat your m onthly pre mi um for Part B will be, bas ed on your income. If you have a higher income, your Part B and Part D premi ums may be higher. This is known as the Income - Related Monthly Adjustment Amount (IRMAA). Your Part B Medicare Costs. Effective January 6, 2025, SSA will require customers to schedule an appoi ntment for service in their field offices, including requests for Social Securit y cards. SSA encourages customers to become accustomed to: • online services, where many transactions can be completed conveniently and securely, and • automated services availabl e on the Nat ional 800 # at 1- 800 - 772 - 1213. Customers who are not abl e to handle their business online or wit h the automated options, m ay call their local Social Security office or the Na tional 800# to schedule an appointment. SSA will not turn people aw ay for service who are unable to make an appointment or do not want to make an appointment. For example, members o f vulnerable populations, military personnel, people with ter minal illnesse s, and indivi duals with other situations requiring immediate or specialized att ention may still wa lk in for service at the field offices. Som e of th e offices also have minimal to no wait times, and they will still serve customers w ho walk in. o 1 - 800 - 772 -1 213; Social Security Website (www.ss a.gov) Page 3 of 20 NO TE: In most situations, Medicare eligibi lity is under a beneficiary’s own social security number. However, in the event the beneficiary is collecting spousal’s/widow/widower benefits, then the Medica re entitlement would be transferred to the reco rd where the beneficiary curr ent ly collects cash benefits. The beneficiary still retain s his/her own unique Medicare number which shows on their Medicare.gov account (see page 8 of this guide). What is Medicare? Medicare is the federal health insurance program that was created in 1965 fo r people 65 & older and som e under 65 with disabilit ies to help with their hos pital and medical coverage. The prog ram helps with the cost of health c are but it is not c omprehensive; it does n ot cover all medical expenses or t he cost of long - term care. Different parts of Medicare help cover sp ecific services Pa rt A – Hospital Insurance Part B – M edical Insur ance Part D – Pr escription Dru g Coverage Preventive Services Medicare does provide numer ous pre vent iv e services at no c ost to beneficiaries (ex. Vaccines, s creenings, etc.) A complete list of these services is a vailable at this link: Medicare Par t B Prevent ive Service s Who is Eligibl e? You are elig ible f or Medicare if y ou are: o 65 years old or older and a U.S. citiz en or lawfully per mitted resident of the United St ates. o Medicare is available for certain people with disabilities who are under age 65. These individuals m ust have received 24 m ont hs of Social Security Disability Insurance (SSDI) benefit payments or ha ve End Stage Renal Disease (ESRD) or Amyotrophic Lateral Sclerosis (ALS). o Most people ar e eligib le for pr emium - free Part A if they have paid Medicare taxes long enough through their own or a sp ouse’s or ex - spouse’s work recor d. If you are w orking and covered by your e mployer’s group health plan (o r by a spous e’s plan), you m ay want to delay enrollment in P art B and enroll only in Part A. You should check with your employer benefits manager on whether or not you need to enroll in P ar t B. The numbe r of employees in your e mployer group health plan may determine if you need to enroll in Medicare as your primary insurance. You can also delay enrollment in Part A unless you are already collecting Social Security benefits. If you have a He alt h Savings Account (HSA) as part of a hi gh deductible employer i nsurance pl an, you may want to delay Part A bec ause you cannot contribute to t he HSA once your Part A coverage begins. You may use money t hat is already present in the account after you enroll in M edicare to help pay f or deductibles, premiums, copayments, or coinsurance. If you contribute to your H SA after your Medicare Part A coverage starts, you may have to pa y a tax penalty. You should stop HSA contributions six months pri or to retir ing. For further HSA questions, refe r to the IRS publication 969, their Hum an Resour ces Depar tment or professionaltax accountant. IRS Publication 969 (2022) Health Savings Accounts Medicare ha s specifi c enroll ment per iods: 1. Init ial Enr ollment Per iod (Part s A, B, C & D) 2. General Enrollment Perio d (Parts A & B) 3. Open Enrollment Period (Par ts C & D) 4. Medic are Advantag e Open Enr ollment Period (MA OEP) (Part C & D- m ust be e nroll ed in an MA plan between Jan 1 st and March 31 st) 5. If you do not enroll during your Init ial Enrollment Period, you may be subject to late enrollment PEN ALTIES (with s ome exceptions), and a possible delay in your coverage. Losing Employer based coverage) Part C (Medica re Advantage) c ombine s Part A, B and usually P art D in one pla n.

T o get Part A and Par t B the month you t urn 65, you must enroll during the first 3 months before the month you turn 65. If you wait until the last 4 months of your I nitial Enro llment Period to enroll, your Par t B coverage will begin t he f irst of the month following the month o f enrollment. * if your birthday fa lls on the fir st day of the mont h, y our coverage would be effectiv e a month earlier Penalties o Part A Late Enrollment Penalty  If you enroll l ate, and aren’t eligib le for premi um - free Part A, your monthly premium may go up 10% for twice the number of years you signed up late. o Part B Late Enrollment Penalt y  If enro lling l ate, Part B penal ty i s a surcharge added t o your monthly P art B premium for li fe. The Part B late enrollme nt penalty is calculated as 10% of the current Part B premiu m for ever y 12 month period you were not en rolled and did not have active employer cov erage. o Part D Enr ollment Penalt y  If you do not have Part D cover age, even i f y ou take no prescr iption dr ugs you can incur a lifet ime penalty. The P art D penalty is c alculated as 1% of the national base beneficiary premi um for each month you were not enrolled in a Part D plan and di d not have creditable coverage. Protectio n f rom Penalt ies o Once you are eligible for Medicare, as long as you are wo rking and covered by your employer’s group health pl an (or by a spouse’s plan), you will not be assessed a Pa rt B Lat e Enrollment penal ty. You will need to prov ide an Employment Let ter to Social Securit y. COBRA does not provide coverage from the Part B penalty. o After you enr oll in Medicare, if you h ave creditable drug coverage fro m any sourc e, including employer, VA coverage, or COBRA coverage, you will not be assessed a Part D lat e enrollment penalty. If you los e this creditable coverage, you will have up to two months to enroll i n a Medicare drug plan to a void any penalties. Two Options fo r Medicare Once you hav e enrolled in Medicare A and B v ia Social Security, you will have two op tions: (See page 9 for a comparison chart) 1) Original Medicar e (Par ts A & B) with an optional Medi gap and/or sta ndalone drug plan (Part D) 2) Medicare Advantage plan (also known as Medicare Pa rt C or MA plan). *You m ust hav e Part A and Part B in order to have a MA pla n. Medicare Advantage Plans Medicare Advantage (a lso known as Medicare Par t C or MA plan) is an “all in one” alternative to Original Medicare (Part s A and B). These bundled plans are of fer ed by private insurance companies that contract with Medicare to provide beneficiaries with all of their Medicare be nefits that include Part A, Part B, and u sually Part D. Key Component s of an MA Pla n: • O ut - of - pocket costs can vary. Some plans may have lower out - of - pocket costs than oth ers for certain services. • W ith Medicare Advantage, you can choose between an H MO, PPO or an HMO - PO S plan. You mus t us e doctors and/or other t ypes of providers who are in the plan’s network if your Medicare Advantage Plan is an HMO (Healthcare Maintenance Organization). You may also need to ge t a refer ral to s ee a specialist. For PPO and H MO - POS plans, you may have the option of choosing ou t of network doc tors but you will usually pay higher co - pays. Ask your primary doctor or other providers you use if they participat e in any Medicare Advantage plans. • Emer gency services w ill be covered anywhere within the United S tates. If you are traveling ou tside your region (zip code/county), check with your plan fo r coverage information. • Most plans offer extra benefits that Original Medicare doesn’t cover, like routine/ limited vision, hearing, and dental. Check with your plan for coverage information. • You may pay a premium for the plan in addition to the mon thly premium for Part B. Some plans hav e no monthly premium. Make sure to check yo ur maximum out - of - pocket cost before committing to any plan. To be elig ible for a plan, you must: o Ha ve bot h Medicare Part A & Part B o Reside in the plan’s geogr aphic serv ice area When can I enrol l or di senroll in a M edicare Advantage Plan? o Initial Enrol lment Per iod o Special E nrollment Pe riod o Open Enr ollment (October 15 - December 7) o Medicare Advantage Open Enro llment Per iod (Januar y 1 - March 31)  Note: You m ust be enrolled in a Medicar e Advantage Plan bet ween January 1 st an d Mar ch 31 st in order to make any changes. Things to consider befo re choosing Medicare Advant age: o Do y our medical providers accept the plan or are y ou will ing to change pr oviders?  PLEASE call all of your providers to conf irm plan acceptance! o How m uch are the premiu m, copays, and coinsurance? o What is the plan’s m aximum out of pocket cost for the year? o Do you need to g et referra ls to s ee a spe cialist? o Ar e your prescr iption dr ugs on the plan’s formular y and what is the cost and are there any restrictions? Medicare M edical Savin gs Account s Not Current ly avail able in Massac husetts Medicare Medical Savings Accounts (MSA) are consumer - directed plans that pair high deduct ible c overage with a Medical Savings Account. Al though these plans are considered Medicare Advantage plans, there a re some important distinctions: o M SA plans do n ot include Part D dr ug cov erage. Individuals who sign up for an MSA would need to join a separate Part D plan to h ave drug coverage. o Ther e are no net works, b ut indivi duals m ust use providers that accept their MSA p lan (See pages 10 - 12 for a Li st of Medi care Advant age Pl ans Currently Offere d in Massachusetts) Medigap Plans Medigap plans, also kn own as supplements, provide ext ra c overage beyond Medicare by f illing s ome of t he gaps in Medicare coverage. Medigap plans do not provide prescription drug coverage. In Massachusetts, there are 7 private insurance c ompanies that offer supplement plans across the stat e. Massachusetts of fer s continuous open enr ol lment, which allows you to enr oll, change or dro p you r plan any month for an effe ctiv e dat e the 1 st of the following month of enrollment. Medigap pl ans in Massachuse tts are also community rated, this means the sa me monthly premium is charged to everyone who ha s the Medigap policy regardless of age or medical history. (See pages 13 - 15 for a List of Medigap Plans Currently Off ered in Massachusetts) Part D Prescription Drug Plans: Even if you do not take any prescr iption medications, you MU ST have a Part D pl an to avoid a lifetim e penalty unless you have other creditable coverage. If yo u h ave a Medicare Advantage plan, mo st include Par t D coverage. If you do n ot have a Medicare Advantage plan, you can enroll in a Medicare Part D Stand - alone Presc ription Drug Plan (PDP). You cannot enroll i n a stand - alone Part D pl an and an Medicare Advant age Prescription Dr ug plan (MA - PD). Beneficiaries with higher incomes will pay more tha n the standard prem ium for either type of plan due to IRMAA. Your Part D P rem ium Cos ts There are 3 phases of Part D drug coverage; 1) Deductible 2) Initial and 3) Catastrophic. Depending on your drug plan, as well as you r annual prescription drug costs, you may not reach all of these phases. Choosing a Part D Pl an: These are t he things to consider when choosing a Part D plan o What is the Total Co st (pr emiums and co - pays)? o Are y our pr escription dr ugs cover ed? o Does the plan have a ded uctible? o Ar e there any restri ctions? (Quantity Lim its, Prior A uthorizati on, Step Therap y, or in/out of networ k) o W hat ph armac ies ar e in - network and are any prefer red? Medicare Prescription Payment Plan The Medicare Prescription Payment Plan is a new payment option in the pr escription drug law that works with your current drug coverage to help you manage yo ur out - of - pocket cost s f or drugs covered by your plan by spreading t hem across the calendar year (January – December). Starting in 2025, anyone with a Medicare drug p lan or M edicare health plan with drug coverage (like a Medicare Ad vantage Plan with dr ug coverage) can use this payment op tion. All plans offer this payment option, and partic ipation is voluntary. If you select this payment option, each m onth you’ll continue to pay your plan premium (if you have one), and you’ll get a bill from your health or drug plan t o pay for yo ur prescription drugs (instead of paying the pharmacy). There’s no cost to participate in the Medicare Prescription Payment Plan. Page 7 of 20
