Summary

The Department of the Treasury is seeking public comment on the extension of an existing information collection, FS Form 133, used for notifying financial institutions of obligations to repay erroneously issued federal payments. The comment period closes on December 1, 2025.

What changed

The Department of the Treasury, through the Bureau of the Fiscal Service, has issued a notice and is requesting public comments on the extension of an existing information collection, specifically FS Form 133 (OMB Number 1530-0003). This form is used to notify financial institutions of their obligation to repay federal payments that were erroneously issued to deceased recipients. The current action seeks to extend this collection, which is estimated to affect over 223,000 respondents with an average burden of 8 minutes per respondent.

Regulated entities, particularly financial institutions, are invited to submit comments by December 1, 2025. The comments should address the necessity, utility, accuracy of burden estimates, and potential improvements for the information collection process. The Treasury is particularly interested in feedback on minimizing respondent burden through automation and assessing associated costs. This is an opportunity to influence the ongoing collection of information related to the reclamation of federal payments.

What to do next

  1. Review FS Form 133 and its associated collection requirements.
  2. Submit comments to the Bureau of the Fiscal Service by December 1, 2025, addressing the utility, accuracy, and potential improvements of the information collection.
  3. Assess the impact of the collection on business operations and potential burden reduction strategies.
