What changed
The NRC is seeking public comment on the renewal of NRC Form 748, 'National Source Tracking System Report,' an existing information collection approved under OMB Control Number 3150-0202. The collection requires licensees that manufacture, receive, transfer, disassemble, or dispose of nationally tracked sources to report transactions through the National Source Tracking System. The estimated annual burden is 2,181 hours across 977 respondents generating 17,825 annual responses.
Licensees subject to nationally tracked source reporting requirements should review their current reporting practices and consider submitting comments on any burdens associated with the collection. The renewal does not change existing reporting requirements but maintains the regulatory framework established when the program was first implemented.

II. Background
Under the provisions of the Paperwork Reduction Act of 1995 (44 U.S.C. Chapter 35), the NRC recently submitted a request for
renewal of an existing collection of information to OMB for review entitled, NRC Form 748, “National Source Tracking System
Report.” The NRC hereby informs potential respondents that an agency may not conduct or sponsor, and that a person is not
required to respond to, a collection of information unless it displays a currently valid OMB control number.
The NRC published a
Federal Register
notice with a 60-day comment period on this information collection on December 11, 2025, 90 FR 57490.

Type of submission: Extension.
The form number, if applicable: NRC Form 748.
How often the collection is required or requested: On occasion (at completion of a transaction, and at inventory reconciliation).
Who will be required or asked to respond: Licensees that manufacture, receive, transfer, disassemble, or dispose of nationally tracked sources.
The estimated number of annual responses: 17,825.
The estimated number of annual respondents: 977.
The estimated number of hours needed annually to comply with the information collection requirement or request: 2,181.
Abstract: The NRC's National Source Tracking System (NSTS) is a secure, centralized database used to track the life cycle of certain
high-risk sealed radioactive sources. Licensees are required to report key transactions involving these nationally tracked
sources, including manufacture, transfer, receipt, disassembly, and disposal. These reporting requirements, first established
in 2006 and fully implemented in subsequent years, support a comprehensive radioactive source control program for radioactive
materials of greatest concern. Information collected through the NSTS is mandatory and plays a critical role in enhancing
the NRC's and Agreement States' ability to track sources from manufacture through final disposition. The NSTS strengthens
regulatory oversight by supporting inspections and investigations, enabling timely communication with other government agencies,
and verifying the legitimate ownership and use of these sources.
Authority: 42 U.S.C. 2011 et seq.
Dated: April 21, 2026. For the Nuclear Regulatory Commission.
