What changed

The Department of the Air Force is proposing to extend its approval for the Air Force Safety Automated System (AF Form 978), a collection of information used for mishap reporting. This extension is necessary for compliance with federal Occupational Safety and Health Administration regulations and Department of Defense policy. The agency is seeking public comment on the necessity, utility, accuracy, and clarity of the information collection, as well as ways to minimize respondent burden. The estimated annual burden is 200 hours for 200 respondents.

Regulated entities, specifically businesses or other for-profit entities and individuals or households that may be involved in reporting mishaps, should review the proposed extension and submit comments by March 16, 2026.
